DOH Issues DAL Announcing Model Residency and Admission Agreements and Addenda

Last week, the Department of Health (DOH) issued Dear Administrator Letter (DAL) #22-28 for adult care facilities (ACFs), which provides links to Model Residency or Admission Agreements or addenda pertaining to the category of licensure the provider has. Moving forward, the Department wants entities seeking licensure as assisted living residences (ALRs) to adopt the ALR Model Agreement consistent with their base licensure, unless the process has already begun using a substitute agreement. DOH hopes that this will streamline the review process. Please see the DAL for additional details. Below are links to each of the Model Agreements:
