EHP Addendum to ISP for ALRs

Through its participation in a work group to streamline Assisted Living Residence (ALR) requirements, LeadingAge NY has secured a change in the Enriched Housing case management and functional assessment processes.  One of the concerns LeadingAge NY raised with the work group was the duplication inherent in conducting case management and functional assessment duties, in addition to the Individualized Service Plan (ISP). We are happy to report that DOH has issued a Dear Administrator Letter (DAL) announcing that Enriched Housing Programs (EHPs) may elect to use the ISP with an EHP addendum to satisfy the requirements for both the EHP case management evaluation and the functional assessment.  EHPs are required to complete both upon admission, annually thereafter and whenever there is a significant change in the resident’s condition.  For more information, click here to view the DAL.  For an example of an ISP with EHP addendum, click here.
