DOH Updates ALP Medical Evaluation and Process

DOH has revised the Assisted Living Program (ALP) Medical Evaluation DSS-4449C as part of the state-wide implementation of the Uniform Assessment System for New York (UAS-NY).  The UAS-NY is being phased in as the assessment tool for the ALP by region.  Click here for the DAL explaining the change and here for the revised DSS-4449C.

The UAS-NY will replace the following ALP forms upon implementation:

  • PRI
  • DSS-4449B Identifying Information
  • DSS-4449D Nursing/Functional/Social Assessment
  • The DSS-4449C Medical Evaluation will be partially replaced by the UAS-NY; the physician's section has been shortened substantially.

Please note that this revised Medical Evaluation form should not be used until the ALP is utilizing the UAS-NY.  In fact, this new process will entail sharing the UAS-NY with the physician, thus the ALP must ensure that the assessment is completed before the individual sees the physician.
