DOH Announces SN-ALP Awards
The Department of Health announced awards for the Special Needs Assisted Living Program grant opportunity this week. These funds stem from the Medicaid Redesign Team (MRT) Affordable Housing Workgroup initiatives, which seeks to make investments that ultimately save Medicaid dollars. LeadingAge NY is on the MRT group and advocated for this funding, which will support activities that help assisted living programs (ALPs) better serve certain special populations better, thereby saving Medicaid dollars. For the purpose of this RFA, the special needs population is defined as Medicaid beneficiaries who have one or more co-morbid and medically complex chronic conditions that are substantially disabling or life-threatening, have a high risk of hospitalization or other significant adverse health outcomes and require specialized delivery systems across domains of care.
