DOH Announces Healthcare Personnel Influenza Vaccination Report to Open April 1st

The Department of Health (DOH) has announced that the Healthcare Personnel Influenza Vaccination Report will open on Thurs., April 1, 2021 and must be electronically submitted to DOH by June 1, 2021. All facilities covered under New York Codes, Rules, and Regulations, Title 10, Section 2.59 (10 NYCRR Section 2.59) are required to document the number and percentage of personnel vaccinated against influenza for the current season and to complete this report upon request of DOH. This includes nursing homes, hospice, and home care providers, including home care associated with assisted living programs (ALPs). Click here to view the Dear Administrator Letter (DAL).
