ALPs Vaccination Reporting
The Department of Health (DOH) has designated that the Healthcare Personnel Influenza Vaccination Report period was opened on Nov. 2, 2015, and will continue through May 1, 2016. Assisted living program (ALP) members are reminded that the requirement applies to all personnel employed under the home health care component of your operations. Providers are required to document the number and percentage of personnel vaccinated against influenza for the current season and to complete this report upon request of DOH.
This year, DOH is requiring that agencies submit a single, longitudinal report that will remain open during the reporting period. DOH has issued a detailed Dear Administrator Letter (DAL) with complete instructions and Frequently Asked Questions on vaccination reporting. DOH notes that the name of the survey form in HERDS is dated November 2014. This is NOT a mistake; it represents the date of the last form update. Directions to access the survey are available on the HERDS Main Menu; and additional information, including contact info for support, can be found within the survey form.
