LeadingAge New York Releases Home and Community-Based Service Options Reference

LeadingAge New York has released a new, user-friendly reference for discharge planners and care managers, entitled “Home and Community-Based Service Options for Seniors and Adults with Disabilities.” It provides a matrix of services available in New York State, along with information about eligibility and reimbursement, and links to more detailed material and local sources. It is intended to serve as a practical and convenient resource for discharge planners and care managers seeking information about appropriate services for their patients, residents and/or members.

The matrix covers a broad array of services for adults with physical disabilities, brain injuries, dementia, post-acute care needs, and/or functional limitations associated with aging. The matrix will be updated periodically and is available on our website by clicking here

This new reference is a companion piece for LeadingAge New York's recently-released publication, "Advancing the Triple Aim through New Models of Care and Payment: A Resource Guide for Home and Community-Bazsed Service Providers in New York State." With these two resources, HCBS providers and their partners along the continuum will have a strong foundation for collaborations that provide high-quality care to individuals in their homes and community settings.
