Age-Friendly Health Systems Action Community Launches

Teams from across the state will be connected to accelerate adoption of the 4Ms — What Matters, Medication, Mentation, and Mobility framework into their practices. The Action Community will provide virtual trainings by expert faculty and monthly webinars to test and implement changes that can improve quality of care for older adults and incorporate age-friendly concepts into care delivery.

The Adult Day Health Care Council (ADHCC) represents 130 medical model adult day health care (ADHC) programs, more than 90 percent of ADHC programs operating across New York State. ADHCC members serve more than 10,000 New Yorkers annually. We are the only organization that speaks exclusively for ADHC providers; we are dedicated to the continuation and expansion of adult day health care in an effort to meet the needs of the frail elderly, chronically ill, disabled adults and their caregivers. We offer our members outstanding operational and regulatory guidance along with state and federal advocacy and an array of educational events, useful products and resources.
