State to Issue New Reporting Forms to ADHCs and ALPs for ARPA Spending
(Jan. 14, 2025) The Department of Health (DOH) communicated this week with adult day health care (ADHC) programs and Assisted Living Programs (ALPs) and shared that they are moving to a new reporting form for the spending of American Rescue Plan Act (ARPA) funds. Their communication is below.
Please let LeadingAge NY know if there are questions or concerns regarding the new reporting tool. Please note in the statement below that DOH will require providers to email DOH by Feb. 1, 2025 to request an extension for spending if they are unable to spend the allotted funds by July 31, 2025.

October 1-December 31, 2024, Reporting
This email serves as an update to the Quarter 2 reporting required for ARPA spending. We are pleased to announce we will be moving the surveys to Alchemer, an online survey questionnaire. This survey platform mirrors the Drupal survey and provides an easier reporting mechanism for providers.
Surveys will be released via email to providers on January 15th, 2025.
Surveys are due February 4th, 2025, by 5 p.m.
With this update:
The survey link will be sent directly to providers via email.
Documentation, such as an updated spending plan (if applicable) and/or proof of spending, can be uploaded directly via the survey.
There will be no need to upload to the Health Commerce System (HCS).
The online survey will walk you through completion and provide you with a copy of your survey responses once submitted.
There will be a limit to how many files can be uploaded (5) so please consolidate your documents accordingly to meet those parameters. **Consolidate is combining your documentation into one file for upload**
All providers must complete the online survey, even if no funds were spent during Quarter 2. The demographic information about the organization and contact information will need to be provided, as well as an explanation as to why no funds were spent. Once this has been completed the survey can be submitted.
If any changes have been made to your spending plan since it was last submitted to the State, please upload the updated spending plan with a summary of the changes as part of your Quarter 2 survey response. These will generally not need prior approval from DOH. You should assume the plan is approved unless you hear otherwise from the DOH ARPA Team.
Please ensure prioritization of any remaining spending in the third and fourth quarters. If you know you are unable to spend all the ARPA funds by the July 31st deadline, please submit an email to the ARPA team at: ALP_ARPA@health.ny.gov or ADHC_ARPA@health.ny.gov no later than February 1st, outlining specific reasons why. Determinations of extension will be on a case-by-case basis.
