ARPA Spending Surveys for First Quarter Posted for ADHC and ALP

(Oct. 17, 2024) The State Department of Health has finally posted the survey templates for reporting ARPA funding for Assisted Living Programs (ALP) and adult day health care (ADHC) providers.  The documents allow programs to submit first quarter reporting and corresponding documentation for spending of funds from July through September 30, 2024. 

The Department has extended the deadline for submission of the surveys to October 30, 2024, 5 pm.  The ALP ARPA survey is here.  The ADHC ARPA survey is here. Please consult these surveys for the process to submit information to the Department, as well as required general information, valid documentation, and spend down categories.  Note that amendments to spending plans, including revised funding amounts or timelines, must be submitted to the Department.
