DOH Announces Application for Statewide Health Care Facility Transformation Program III
The long-awaited application for the Statewide Health Care Facility Transformation Program III funds has been released. Please click here for the funding announcement from DOH.
Long term care providers are eligible for this funding, but awards have historically gone to acute and primary care providers. Nursing homes, adult care facilities, home care, and hospice providers are among the providers that are eligible to apply for the funding. See the below for a brief background and timelines, including an upcoming webinar on Oct. 14th and the deadline for questions on Oct. 28th.
Background:
The objective of the Statewide Health Care Facility Transformation Program III is to provide grants in support of capital projects, debt retirement, working capital, or other non-capital projects that facilitate health care transformation activities including, but not limited to, merger, consolidation, acquisition, or other activities intended to create financially sustainable systems of care, preserve or expand essential health care services, modernize obsolete facility physical plants and infrastructure, foster participation in alternative payment arrangements, increase the quality of resident care or experience for residential health care facilities, or improve health information technology infrastructure, including telehealth, to strengthen the acute, post-acute, and long term care continuum.
Timelines:
Applications must be submitted in Grants Gateway by 4 p.m. on Wed., Jan. 12, 2022. For application information, visit the DOH website at http://www.health.ny.gov/funding/.
