The Centers for Medicare and Medicaid Services (CMS) developed this Health Care Provider After Action Report/Improvement Plan (AAR/IP) template to provide a voluntary, user-friendly tool with an organized, thorough approach for gathering details on emergency preparedness exercises and real emergency events to identify areas that may need further improvement.  Completion of the CMS AAR/IP template meets any CMS exercise documentation requirements.

This website allows visitors to download the full manual and guide for the Nursing Home Incident Command System (NHICS).   NHICS assists providers in planning for an emergency response based on the Incident Command System (ICS), a standardized, on-scene, all-hazards incident management structure that accommodates emergencies of all type, scope and complexity.  NHICS uses ICS principles and practices specifically adapted for the long term care community.  ICS is used on the federal, state and county levels and is the basis for New York's Emergency Management System.
