DOH Activates ACF Financial Report for 2013

The Department of Health has activated the 2013 Adult Care Facility (ACF) Financial Report on the Health Commerce System.  For facilities using a calendar year (Jan. 1 to Dec. 31) as their fiscal year, the due date is Dec. 1, 2014.  For facilities that use a fiscal year period that differs from a calendar year, the report must be filed by the 15th day of the sixth calendar month following the close of their fiscal year.  Click here to access the Dear Administrator letter, which providers further details and instructions regarding the reporting.  Questions can be directed to DOH’s Nancy Nowakowski at 518- 408-1624 or acfFinRpt@health.state.ny.us.
